This is due, at least in part, to a multi-faceted approach that goes beyond the rehab center to include home evaluations prior to discharge and a strong focus on developing relationships with home care agencies as well as equipment and service providers within the community.
When it comes to rehab, patients can expect expert care as well as accessibility and scheduling flexibility from a professional staff consisting of Occupational Therapists, Physical Therapists, a Speech Therapist, and on-site support personnel.
In addition to providing high-level therapeutic care, the rehab associates are also actively involved in the discharge planning process, and even make home visits to assess safety levels, identify areas of concern and suggest appropriate adaptations. Recommendations are then made based on comprehensive input from the therapist, a social-services professional and the nurse manager, thus increasing the likelihood of a “safe and successful” return home after rehabilitation.
